Varsity Vipers Basketball Club

I, as the parent/guardian of the member whose information is given above, or as a participant player over the age of 18 years, consent to the player becoming a registered member of the Varsity Vipers Basketball Club and expressly agree to be responsible for the member's behaviour and accept the terms set out in this membership declaration.

The member is medically and physically fit and able to participate in the basketball activities. I will immediately notify the Club of any change to the member's medical condition, fitness or ability to participate. I understand and accept that the Club will continue to rely upon this declaration as evidence of the member's fitness and ability to participate.

I consent to receiving the member any medical treatment that the Club reasonably considers necessary or desirable during the member's participation in basketball activities. I also agree to reimburse the Club for any costs or expenses incurred in providing the member with medical treatment.

I acknowledge and consent to photographs and electronic images being taken of the member during my participation in any basketball activities. I acknowledge and agree that such photographs and electronic images may be used by the Club for promotional or other purposes without my further consent being necessary.

I understand that the information I have provided above is necessary to provide the member with membership services and for the general business of the Club. I understand that I will be able to access the information by contacting the Club at info@varsityvipers.com.au. I acknowledge that the Club may also use my personal information for the purposes of providing the member with promotional material from Club sponsors or third parties.

I understand that I can access this Privacy Policy of the Club at any time.

Participation in any Varsity Vipers Basketball Club activity is undertaken at the member’s own risk.
While the Club takes all reasonable care in the conduct of its programs, training, and events, it accepts no responsibility for injury, loss, or damage to persons or property that may occur before, during, or after participation. Members and their guardians acknowledge that basketball is a physical sport involving inherent risks and agree that they voluntarily assume all such risks.

The Club strongly recommends maintaining appropriate personal accident and health insurance coverage.
